An incident may be defined as an unwanted and uncontrolled event (or sequence of events) that disturb normal operations and has caused or could have caused negative impact on:

• Safety and health of humans
• The environment
• Material, like the vessel or structure
• Product, like cargo quality or charter performance
• The reputation of the organisation

Reporting Incidents

The company upholds a NO BLAME POLICY for reported incidents. Information from reported incidents will be used for analyses to prevent re-occurrence of a similar incident and will not be used to blame a certain individual who just tried to do his job.
Lauranne Shipping uses LS intra to report and record incidents in the fleet. The incident reporting forum is an intelligent interactive form that asks to enter certain details, dependent of previous answers. It allows us also to have quick overview of statistical information of a certain year.
Incident information is passed on to the cargo owner, flag state, Class and to other stakeholders who are monitoring our performance, like charterers and oil major clients.

Root Cause Analyses

Incidents are investigated to get the facts straight, to analyse what exactly happened and then to define what action can be taken to further limit the risk of re-occurrence.
To systematically analyses causes and contributing factors and to evaluate the effectiveness of existing safeguards a Root Cause Analyses is performed.

The investigation report accompanied by defined actions is sent to all stakeholders involved.
The actions are entered in the companies ActionTracker to be able to ensure and to monitor the completion of the action.
Lessons learned from incidents are posted in the ISM Contact Forum in LS intra. Items posted in the ISM Contact Forum are discussed in monthly Safety committee Meetings on board.
